Interviews

 

To become a client of The Anabolic Clinic ^sm, you must first complete a questionnaire.

 

Then you will have an interview. Interview fees are non-refundable. If you are overweight or obese, please go here for additional information.

Or, you can come to our office and pick one up.

 

Complete the questionnaire in its entirety and return it to us.

 

 If the returned questionnaire is incomplete, we will not schedule an interview.

 

After we receive the completed questionnaire, we will review it.

 

If we determine that it is appropriate for us to work with you, we will contact you and schedule your interview.

 

If we determine that it is not appropriate for you to become a client of The Anabolic Clinic ^sm, we will contact you to let you know. We will not schedule an interview.

 

Here are some examples of why we would not agree to work with you.

 

There are some conditions for which anabolic substances are contraindicated (inadvisable).

 

If you have any of these conditions, do not submit a questionnaire or the other material:

• If you are a man with a history of untreated prostate cancer or breast cancer, currently available anabolic substances are contraindicated. We will not accept you as a client.
• If you are a woman with a history of breast cancer, are trying to get pregnant or have Polycystic Ovary Syndrome (PCOS) currently available anabolic substances are contraindicated. We will not accept you as a client.
• If you are overweight or obese and want to become a client of The Anabolic Clinic ^sm, please go here.

 

Following your interview, certain tests may be performed. For example, we may require a mammogram or PSA test.

 

If these tests indicate that you have a condition where currently available anabolic substances are contraindicated, e.g., prostate or breast cancer, we will not be able to work with you.

 

If you are aware that you have these conditions and they may be discovered when the tests are performed, do not seek to become a client of The Anabolic Clinic ^sm.

 

Once again, interview fees are non-refundable, even if the testing reveals a condition of which you were unaware at the time of your interview and that prevents us (in our sole discretion) from working with you as a client of The Anabolic Clinic ^sm.
